week 6

My supervisior told me that I am really getting the hang of the documentation-that made me feel good. They use narrative style documentation at this site. This week  the clients have finally got all of their ideas together for the monthly newsletter. The newsletter is something new for the clients, and this is the first month that they will be putting out one- they are very excited. Each client gets to decided on something that they would like to put into the newslerrer and then it is approved. I must say I really enjoyed working with the different clients to get their parts on paper for the newsletter.

 

 

 

Progress Note:

I really am understanding the documentation process now. I believe I am learning more of how to approach a situation may it arise verses two weeks ago. I am really starting to enjoy myself at this site more this week.

week 10 # 8

S. My assisatant supervisor said that I was a great student to work with. She also added that if I would like to be a sub. for them that they would love that, and that I could be put on teh sub. list.

O. This week I had to say my good-byes to the clients. I really enjoyed my experence at the facility.

A. I hope that the clients improve on their goals, some of them really have a long way to go- I wish them the best.

P. My observation hours are done, as of Tuesday. I plan to be a sub. for the facility when I can.

Where shuld I start? A very interesting week. I was told off again by the same person you might of reaad about two weeks go. My supervisor told me to not take it personal. I have not taken it personal, but I think she will be thankful when mu time there is done.

Next week we will be getting out our monthly newletter. The clients have each decided on a part of the paper that will be their responsibility for. They will be bringing in their part on Thursday  and begin to put it together. I am to start getting a something together for the intergration/communication group to teach in group for the following Thursday. I am a little nervous about that, but it will only be for an hour, then my supervisor she I could step it up.

 

 

Progress Note,

I have now done three progress notes and the last one that I had done they did not change anything about it- which made me feel some sort of accomplishment. I fell it will get better as the time goes by.

This week the clients activity was exercise. They picked out of a magazine an exercise that we could all do in the room . Everything was going fine and well. I help and moved around the room and gave anyone extra help that may have needed it. After they had found their exercise and had it aproved by Kelley they were to glue to a piece of construction paper. Everyone was at a did place in their activity. I asked J. if she needed any help because I had noticed that her construction paper was blank and that she had no picture cut out yet. She instantly got iritated with me and said NO I have been asking for help all this time and eveyone has been ignoring her. This was not true at all. I just happened to say the wrong thing at the wrong time. J. decided she was done with group and gathered her things and went outside. I felt awful, as if it was my fault- I am still learning. After everyone was done with their activity J. was asked only once to come back in and “process” what had happened. Kelley told her what she saw and J. agreed. J. said she had a few triggers , which she had shared some of them during goal reveiw. Kelley made a good poit after the process was over. That if a client is having a extra bad day, let them know, it was let known to them that if could be done in private. Had we of known J. day was that bad before she had gotten there Kelley told them we would of started helping people on the other side of the table.

 

 

Progress Note:

This week I took my own notes and put the documentation into the computer I would say 90% myself, so I am making progress with the documentation. I find that documentation has to be worded in such a way that you are not making it sound like it is something that the client did not directly say.

Week 3

This week I have been sitting in with a group on Thursday mornings. This group not only has a chemical dependencey, but also a mental illness. It is a small group. One client is only 22 or 23 and has been drinking since he/she has been 13. Two clients inparticularly caught my interest; after sitting thought group, I than realized why. They both had gave up their children due to drugs. The one patient  mentioned that he/she was a functional alcoholic for years. As I am listening to the patient barely being able to breathe with oxygen, whizzing,barely able to clear his/her throat when trying it all started to become a little clearer. I found out that she/he had became a crack addicit for sometime, at this point she/he decide on their own to give their children up for adoption because he/she could no longer take care of them. The client said it was the hardest thing she/he has ever done , but the drugs had control of him/her. This client is not allow to try ande contact the children til they are the age of 21.

Another client put her children in foster care due to being a “coke” addicit. The client does not know where they  are and said he/she does not think that they will ever want anything to do with him/her again.

I wish all the clients the best and hope they can continue to stay “clean” and can live healthy lives.

 

 

Progress Note,

Kelly is starting me to take notes in shorthand as if I were the one to be filling out the documentation for the client. I took notes than she and I compared mine to hers. Kelly theen gave some feedback about what was good about them and what I could of added or missed by what a client had said. It was another good week.

Stacey’s Experience

Started and finished first week with getting quite familar with my supervisor. We took a quick tour of the building, and Kelly mainly showed me where our paperwork would need to go in the main office. I made a few copies. Met several of the patients from one disorder to another. Some of the patients, it was unbelievable to see the suffering of the mental illnesses that they have to go through on a daily basis. I am looking forward to next week.

 

Intial Note

Met with my supervisor, got familar with the filing system, and started to get faces with names of the patiets.
